Primary Care visits are conducted by Autonomous Advanced Practice Nurse Practitioners
~ Primary care visits in the residential, Independent Living, Assisted Living, Adult Care and Group home setting, Telehealth also available.
~ Management and coordination of acute and chronic medical conditions for those individuals with limited access to health care services, limited or inability to leave the home due to debilitating physical or mental limitations.
~ Initiate Home Health Physical, Occupational, Speech Therapy, Nursing Services.
~ Order Durable Medical Equipment
~ Medication management and ordering prescriptions
~Refer to specialists including pain management and hospice as appropriate.
~ Coordinate in-home lab draw, diagnostic imaging and ultrasound.
~ Assessment / Form completion: 1823, 3008, VA forms, FMLA, Disabled parking permit, LTC insurance forms, etc.
~ Pre-employment Physical, TB Screening, Communicable disease statement.
